Apply to Relocate Your Pharmacy

Pharmacists-in-charge of licensed pharmacies that plan to move from one location to another with no change in ownership, whether that location is at a new address or within the existing premises with no address change, can apply to do so through their account in the CPNL Pharmacy Portal at least 90 days before relocating the pharmacy.

REQUIRED DOCUMENTATION

The following documentation is required to be submitted with the Pharmacy Relocation Application.

Application

If you have obtained all the required documentation, you may apply through the College’s online pharmacy portal by following the steps below.

Click Pharmacy Portal in the Portal Login menu at the top right of the webpage.
Log into your account and, if more than one pharmacy profile is linked to your account, click the Select button next to the appropriate pharmacy profile.
Click the Pharmacy Relocation Application link on the side menu.
Click the Start button under the Pharmacy Relocation Application table.
Follow the instructions to submit the application and pay the application fee.

Completed applications must be submitted at least 90 days before relocating the pharmacy and may take 7-10 business days to process once all required documentation and fees are received. Applicants will be notified by email once a decision has been made. Approved applications are subject to a successful inspection before reopening at the new location.

Pre-opening Requirements

Notify the public, through advertising and/or signage, of the new location of the pharmacy and the date of the location change.
Schedule and participate in a pre-opening site visit with the College’s Community Pharmacy Practice Consultant or Hospital Pharmacy Practice Consultant.
Complete and Submit a Pharmacy Self-Assessment at least one week before the scheduled pre-opening site visit.

Connect to the Pharmacy Network.
Remove signs from the previous location upon relocation of the pharmacy.
