Request a Name Change

Registered pharmacy professionals can update their contact, employment, and education information through their account in the CPNL Registrant Portal. However, for registered pharmacy professionals to make a change to their name on the register, a request must be submitted to the College with supporting documentation.

Required Documentation

Updated photo identification must be provided with the application. Acceptable forms of identification include a passport, provincial or territorial driver’s license, or provincial or territorial photo identification record. Other identification may be acceptable as long as it meets the following requirements.

Must be valid and current.
Must have been issued by a federal, provincial, or territorial government, or by a foreign government in the case of international applicants.
Must include a clear photo of the applicant, their legal name, and their date of birth.

Application

If you have recently changed your legal name please submit a name change request through the College’s online registrant portal by following the steps below.

Click Registrant Portal in the Portal Login menu at the top right of the webpage.
Log into your account.
Click the Name Change Request link in the side menu.
Follow the instructions to submit the request.

Requests are reviewed in the order they are submitted and may take 3-5 business days to process. Applicants will be notified by email once the request has been processed and the name has been updated on the register.

Pharmacists-in-charge who request a name change must re-reprint the pharmacy licence certificate with the revised name once the request has been processed and approved.
