Apply To Change Pharmacy Name

Pharmacists-in-charge of licensed pharmacies that plan to change the pharmacy operating name with no change in ownership can apply through their account in the CPNL Pharmacy Portal.

Application

If you plan to change the pharmacy operating name please submit an application through the College’s online pharmacy portal by following the steps below.

Click Pharmacy Portal in the Portal Login menu at the top right of the webpage.
Log into your account and, if more than one pharmacy profile is linked to your account, click the Select button next to the appropriate pharmacy profile.
Click the Pharmacy Name Change Application link in the side menu.
Click the Start button under the Pharmacy Name Change Application table.
Follow the instructions to submit the application and pay the application fee.

Requests are reviewed in the order they are submitted and may take 5-7 business days to process. Applicants will be notified by email once the application has been processed and the name has been updated on the register.

Pharmacists-in-charge must re-reprint the pharmacy licence certificate with the revised name once the name has been updated on the register.
